  • Scheduling Meetings
    To book a meeting with your physician, please send an email to . Please let us know if you'd like an in person meeting or a phone call. To book a meeting with your coach, please use the following link :
  • Cancellations, reschedulling, and missed appointments.
    We recognize that life can be unpredictable. Late Arrivals: While we do our best to accommodate late arrivals, your promptness is appreciated. In the event you anticipate being late, please email us in advance. Please note that late arrivals may necessitate rescheduling your appointment to ensure we can provide you with the quality care you deserve. Cancellations or Rescheduling: If you need to cancel or reschedule your appointment, kindly provide us with a minimum of 24 hours' notice. This allows us to accommodate other patients effectively. Please be aware that late cancellations or no-shows may incur charges as outlined in our fee schedule. Rescheduling late or missed appointments will be subject to our fee schedule. We encourage you to communicate any scheduling changes with us as promptly as possible.
  • Fee Schedule
    Missed in-office physician appointment: $300 Missed physician phone call: $100 Missed coaching intake meeting: $150 Missed follow-up coaching meeting: $100 Payments can be made via EMT to and will not be rebooked until payment is recieved. Please contact us in order to re-book and discuss payment options.

  • Appointment cost and insurance coverage
    We direct bill AHS for all meetings with physicians and coaches. Coverage for bio-trackers, recommended devices or, equiptment will be subject to your insurance.
  • Prescription Refills and Medication Policies
    We provide medication management to ensure the safe and effective use of prescribed medications. If you have concerns about your medications, please discuss them with your healthcare provider. You can request prescription refills by contacting us at . Please allow sufficient time for processing to avoid interruptions in your medication regimen.
  • Patient Privacy and Confidentiality
    We take your privacy seriously and adhere to strict confidentiality standards. Your medical information is securely stored and accessible only to authorized personnel. We use your medical data for the sole purpose of providing you with the best possible care. If your information is shared with other healthcare providers or for research purposes, we will seek your consent.
